Gulf Pointe Plaza Skilled Nursing Facility in Rockport, Texas, offers a plush residential environment, and at the same time, meets all Federal and State guidelines. Our Nursing Facility Team offers the highest possible level of medical services.

Our state of the art Medicare Unit provides Specialized Rehabilitation and Medical Services for those residents that have had a recent illness or injury.

Following rehabilitation, the residents may choose to stay at Gulf Pointe Plaza or return to their home. Residents may also elect to move next door to Gulf Pointe Village, a licensed Independent and Assisted Living Residence.

We offer the best services available in our licensed 120 bed Skilled Nursing Facility located on Enterprise Boulevard. Our facility offers a homelike atmosphere for the comfort and well-being of our residents.

Our Care Team assesses each potential resident before admission to assure that the physical and psycho/social needs can be met by our facility. Our Interdisciplinary Health Care Team monitors the resident's physical and psycho/social needs. 
  
The trained professionals of the Health Care Team design a specific plan of care for each resident following a comprehensive assessment. This allows resident the chance to obtain their highest physical and psycho/social level of functioning.
